Diary Cards

Diary cards are an important part of DBT. It allows you to record emotions on a daily basis, along with any DBT skills you may have used. Drug Court uses the app PSYCH SURVEYS to capture Diary Card information. In order to be set up on the PSCYH SURVEY app, you need to provide staff with an email address you can access. Once set up, you will receive an email with your password. You are able to change your password to something easier to remember once you sign in for the first time. Below is a sample of what the diary card looks like. If there is anything you do not know (such as skills you haven't learned yet), you can leave it blank. EMOTIONS and THOUGHTS should be filled in everyday, but the skills can be left blank until you start to learn them.

If the above is not done, it will be marked and incomplete. The standard response for failing to do a diary card is 1 hour of community service for each diary card (for phases 1 & 2) and extra treatment days for phases 3-5.
